Position: Cook
Job Type: Part-time
Shift: Day Shift (8 hour shifts & rotating weekends)
Compensation: From $15.00 per hour (based on experience)
Work Location: In person at 115 Main St. Sidney, IA 51652
Duties:
- Complete food preparation tasks
- Cook and prepare meals according to recipes
- Cooperate in keeping the entire kitchen clean and in order at all times
- Report equipment repairs needed to the Dietary Manager
- Prevent food waste
- Pour juices and beverages and serve on trays at correct temperature, neatly.
- Prepare and serve salads, fruits, dry cereals and cold breads.
- Wrap flatware in napkins
- Set up trays for meals
- Assist with tray service to residents in the dining area
- Be aware of resident nutrition needs or restrictions and follow all care plans

Position: RN (Registered Nurse) Home Health
Job Type: Full-Time, Part-time & PRN available
Shift: Day Shift (Primarily Mon-Fri. Weekends as needed. Schedule is flexible)
Compensation: $40-$45 per hour
Work Location: Home health travel in the Lincoln area
Duties:
Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
Initiates the plan of care and necessary revisions.
Initiates appropriate preventive and rehabilitative nursing procedures.
Informs the physician and other personnel of changes in the condition and needs of the patient.
Counsels the patient and family/significant others in meeting nursing and related needs
Determines the amount and type of nursing needed by each individual patient.
Conducts patient care conferences on patients assigned to his/her care.
Gives total patient care as needed.
Takes on-call duty nights, weekends and holidays, as assigned.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
Works diligently with the Director of Clinical Services to ensure correct information during coding and OASIS reviews.
Requirements:
Graduate of an approved school of professional nursing and currently licensed in Nebraska.

Position: PTA (Physical Therapist Assistant) Home Health
Job Type: Full-time, Part-time, or PRN available
Shift: Day Shift (Primarily Mon-Fri. Weekends as needed. Schedule is flexible)
Compensation: Compensation based on experience
Work Location: Home health travel in the Omaha area
Duties:
Provides physician prescribed physical therapy.
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive level within the patient's capabilities.
Takes initial history and makes initial evaluation.
Consults with physicians regarding change in treatment.
Instructs patients and family/significant others in home programs and activities of daily living.
Prepares clinical and progress notes.
Assists physician with evaluating level of function.
Helps develop the plan of care and revise as necessary.
Completes and submits OASIS assessments, reassessments at least every 60 days, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames an in accordance to 175 NAC 14-006.04H1.
Appropriately utilizes ICD-10 codes.
Requirements:
Nebraska Physical Therapy License (Required)
Willingness to obtain Iowa Physical Therapy License upon hire is a plus

Position: Admissions Nurse/Medical Records
Job Type: Full-Time
Shift: Day Shift (Monday through Friday)

Duties:
- Assure complete medical records program is maintained for resident, clinicians, and billing
- Oversee MCO authorizations and weekly updates, nursing daily schedule and long term care insurance
- Conduct comprehensive assessments of patients upon admission
- Collaborate with healthcare team members to develop individualized care plans
- Coordinate and facilitate the admission process, including completing necessary paperwork and obtaining patient information
- Provide education to patients and their families regarding their condition, treatment options, and available resources
- Assist with discharge planning and ensure appropriate follow-up care is arranged
- Administer medications and treatments as prescribed by physicians
- Monitor patient progress and report any changes or concerns to the healthcare team
- Maintain accurate and up-to-date medical records
Skills:
- Strong knowledge of hospital admission procedures and protocols
- Excellent assessment skills and ability to identify patient needs
- Effective communication skills, both verbal and written
- Ability to work well under pressure and prioritize tasks efficiently
- Attention to detail and strong organizational skills
- Proficient in using electronic medical record systems
Requirements:
- Valid RN or LPN License
- BLS (CPR) Certification
