Position: RN (Registered Nurse) Home Health
Job Type: Part-time or PRN available
Shift: Day Shift (Primarily Mon-Fri. Weekends as needed. Schedule is flexible)
Compensation: $38-$44 per hour
Work Location: Home health travel in the Omaha area
Duties:
Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
Initiates the plan of care and necessary revisions.
Initiates appropriate preventive and rehabilitative nursing procedures.
Informs the physician and other personnel of changes in the condition and needs of the patient.
Counsels the patient and family/significant others in meeting nursing and related needs
Determines the amount and type of nursing needed by each individual patient.
Conducts patient care conferences on patients assigned to his/her care.
Gives total patient care as needed.
Takes on-call duty nights, weekends and holidays, as assigned.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
Works diligently with the Director of Clinical Services to ensure correct information during coding and OASIS reviews.

Position: OT (Occupational Therapist) Home Health Omaha
Job Type: Part-Time
Shift: Day Shift (Monday through Friday. Weekends as needed.)
Compensation: Based on prior experience
Work Location: Home health travel in the Omaha area
Duties:
- Returns the individual to optimum and productive living within the patient’s capabilities.
- Periodically participates with all other health care personnel in patient care planning.
- Provides prescribed occupational therapy which may include: teaching daily living skills; developing perceptual-motor skills and sensory integrative functioning; developing pre-vocational capacities; designing, fabricating, or applying selected orthotic and prosthetic devices or selective adaptive equipment; using specifically designed therapeutic media and exercises to enhance functional performance; administering and interpreting tests, such as manual muscle and range of motion; and adapting environments for the handicapped.
- Takes initial history and makes initial evaluation
- Instructs patients/family members in home programs and fine motor movement exercises.
- An initial evaluation, including plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary, if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. A summary is written upon patient’s discharge.
- Helps develop the plan of care and revises as necessary.
- Prepares clinical and progress notes.

Position: PT (Physical Therapist) Home Health
Job Type: Part-time
Shift: Day Shift (Primarily Mon-Fri. Weekends as needed. Schedule is flexible)
Compensation: Compensation based on experience
Work Location: Home health travel in the Omaha area
Duties:
Provides physician prescribed physical therapy.
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive level within the patient's capabilities.
Takes initial history and makes initial evaluation.
Consults with physicians regarding change in treatment.
Instructs patients and family/significant others in home programs and activities of daily living.
Prepares clinical and progress notes.
Assists physician with evaluating level of function.
Helps develop the plan of care and revise as necessary.
Completes and submits OASIS assessments, reassessments at least every 60 days, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames an in accordance to 175 NAC 14-006.04H1.
Appropriately utilizes ICD-10 codes.
Requirements:
Nebraska Physical Therapy License (Required)
Willingness to obtain Iowa Physical Therapy License upon hire

Position: CNA/CMA
Job Type: Full Time and Part Time positions available; $4000 Sign on Bonus (Full-Time)
Shift: Day, Evening and Night Shifts
Evening Shift staff get an additional $1.00 per hour differential starting at 2:00 pm.
Night Shift staff get an additional $1.50 per hour differential starting at 10:00pm.
Work Location: In person at 1800 14th Ave. Nebraska City, NE 68410
Duties:
- Assist with daily living activities. This includes delivering meals, turning and ambulating patients and providing plenty of fresh water, as well as extra nourishment between meals
- Help patients with personal hygiene. Provide patients with bedpans, help with baths, shampoos and showers
- Provide adjunct care for the patient, including ice packs, non-sterile dressings and therapeutic baths
- Check vital signs and record daily information in the patient's chart
- Assist the nurses and other staff as needed
- Adhere to professional standards, follow policies and procedures and abide by federal, state and local requirements.
Requirements:
- High School diploma or GED
- Completion of a state-approved CNA certification training course
- Basic computer skills
- Ability to work as part of a team and to handle multiple tasks safely and effectively
Good communication skills, including the ability to communicate sensitive information with empathy ("bedside manner") - High tolerance for working under pressure and handling critical situations with a cool head
Certified Nursing Assistant CNA Desired Skills - CPR Certification (preferred)
- CNA License (required)
Experience: Caregiving: 1 year (Preferred)
